It seems Kevin Feige wasn't pleased with Joss Whedon's original direction for the villain.
Although Avengers: Age of Ultron is looked back on as fondly as the first film, it’s hard to deny that James Spader did some excellent work in bringing the eponymous villain to life. But he nearly had a more grounded look instead of his repurposed Stark-tech design. According to one artist, Marvel Studios head Kevin Feige intervened when he saw what Joss Whedon had planned for the villain. Whedon has been in the headlines a lot recently as Ray Fisher continues his campaign to get justice for the cast and crew who were affected by the director’s alleged abusive behavior on the Justice League reshoots.
According to artist Phil Saunders, Joss Whedon wanted Ultron to be filled with gears and pistons to give him more of a realistic look. Saunders explained that the designs were too ‘endoskeleton’ for his taste in the caption of his first photo. The artist went on to reveal more concept art from Avengers: Age of Ultron which shows an earlier look of the grounded style that Kevin Feige didn’t like.
